Title: Exit from game

Body: When a user exits out of a game, instead of returning the user to just the play now page, open up a window that allows the person to: a) submit a quick write-up about the game they where just in. Auto-populate the date/time, and what server they came from (the latter would require in-game modification, so that could be done later). b) give the person the ability to issue kudos to people along with a description. The hope is we'd get more reasonable things out of kudos. c) honorable mention: No kudos associated, but maybe with the quick write-up, there could be a line about people they wanted to mention.
